To apply, simply print out the application, complete it and drop it off at Zone Action Park with your City of Lewisville Food Handlers Card. You must turn it into the Manager on Duty.

The minimum requirements for employment are:

• The applicant must be 16 years old.
• The applicant must have a food handlers card from the City of Lewisville Health Department. Your application will not be accepted unless you have your Food Handlers Card with you when you turn in your completed application.
• The applicant must have two forms of identification from a local, state or federal government agency. At least one must be a picture form of identification.
• The applicant must be available days, nights and weekends.

To get your City of Lewisville Food Handlers Card, you must call the City Health Department at (972) 219-3400 and they will instruct you on the criteria needed to obtain their Food Handlers Card.

*A FOOD HANDLERS CARD FROM ANOTHER CITY OR COUNTY IS NOT ACCEPTABLE!*
